Application for Grant Funds under the Public Health Service Act for Replacement Hospital, Medical Center

(5) T h e Public Health Service Act authorizes federal funds to assist in the construction of new facilities and the expansion, replacement, or rehabilitation of existing schools of medicine, osteopathy, dentistry, public health, pharmacy, optometry, podiatry, and veterinary medicine. I t is possible that funds will be available from this source for assistance in the construction of the replacement hospital at the Medical Center. T h e requirements for the submission of an application for a grant under this act include the adoption of a formal resolution by the governing board of an institution, authorizing the filing of the application, including all understandings and assurances contained therein, and directing and authorizing the person identified as the official representative of the applicant to act in connection with the application and to provide such additional information as may be required. In compliance with these regulations, the Vice President for Planning and Allocation has recommended that the Board of Trustees adopt the resolutions submitted herewith. I concur.

Resolution Authorizing the Filing of Application for Grant Funds under Hill — Burton Program Title VI of the Public Health Service Act

Whereas, the Public Health Service Act authorizes Federal grants to assist in the construction of new facilities and modernization of existing facilities; and Whereas, T h e Board of Trustees of the University of Illinois, the governing body of the University of Illinois, hereinafter called the Applicant, is cognizant of the conditions under which such grants are made available and approved for payment to the applicant; and Whereas, it is the sense of the governing body of the Applicant that it is desirable to apply for a construction grant under the aforementioned act for the project described as follows: Replacement Hospital Facility Now, Therefore, Be It Resolved, that an application for the project described is authorized and directed to be filed with the Department of Public Health, State of Illinois; and be it further Resolved, that The Board of Trustees of the University of Illinois designates James E. Osbom, Director of Business Affairs of the University of Illinois at the Chicago Campuses, as the officer authorized to file the application and act as the representative of this Board in connection with said application. Whereas, the Public Health Service Act authorizes Federal grants to assist in the construction of new facilities and the expansion, replacement, or rehabilitation
